1. I understand and acknowledge that I am of the full age of 18 years or older and I confirm that I am not under the influence of alcohol or any illicit or prescription drugs which would in any way impair my ability to agree to the terms of this agreement or safely commence the procedures herein.

2. I acknowledge that I have been advised by Katie Schofield PMU Ltd. not to drink alcohol, take aspirin or ibuprofen for 48hrs or have caffiene for 24 hours before the procedure. Any permanent procedure (eyeliner and/or eyebrow) are not suitable for someone who is pregnant and/or breastfeeding or type 1 diabetics. I further hereby save harmless and indemnify Katie Schofield PMU Ltd. for any damages whatsoever resulting from me not complying with the request Katie Schofield PMU Ltd. has stated herein.

3. I corfirm that I agree to follow the stated aftercare and understand that my failiure to do so will result in poor results and Katie Schofield PMU Ltd. cannot be held liable.

4.  confirm I do not have hemophilia, Hepitisis, HIV, Diabetes, nor do I take blood thinning medication or any other medication including but not limited to Accutane that may interfere with the procedure, application or healing of any procedure mentioned herein. I also confirm that I do not have any other medical or skin condition that may interfere with the procedure, application, or healing of any procedure mentioned herein.

5.I confirm for eyeliner tattoos I have removed eyelash extensions and stopped Revitalash or similar growth serums for 2 weeks and had Botox or peels no nearer than 2 weeks to the appointment.

6. I confirm for eyebrow tattoos I have waxed, threaded, tinted at least 72 hours before the treatment and stopped Revitabrow 2 weeks prior and had Botox no neaer than 2 weeks to the appointment

7.I confirm for lip tattoos I have taken antivirals as instructed by a medical practicioner or pharmacist if I am prone to coldsores and left at least 6 weeks after having any lip fillers.

8.I confirm for areola tattooing that all surgery was longer than 6 months ago and I do not intend to have further surgery.

9. I understand 1n the first seven days Eyebrows are up to 40% darker and 10 to 15% thicker. Colour reflection depends on the natural skin pigment and that pigment is absorbed differently due to differences in the skin quality, there is no warranty for the treatment success .

10. I understand the shape is determined according to the face proportions.

11, I understand the minimum or maximum duration of Microblading or permanent make up procedures cannot be determined with certainty, nor can any warranty be given on the performed treatment. 

12. Permanent Make up is at least a two part process. No further work can be carried out until the first session is completely healed in 8-10 weeks. 

13. A touchup is included in the price. Touchup fees may apply for further appointments. The touch up has no monetary value and no refunds can be given should you fail to attend. If you take out the payment plan option and don't attend your touch up, you will still be invoiced for it. Clients will not be considered a touch up after a period of three months.
 
Should more touchups be required then the fee may apply.
